Registration Fee Includes:

• Program Booklet

• Breakfast (daily)

• Refreshment Break (daily)

- From the initial confirmation until June 01, 2015, there will be a penalty of $150 of the total Registration Fee.

- After June 01, 2015 and up to July 01, 2015 the penalty will be 50% of the total Registration Fee.

- After July 01, 2015, there will be no refunds for the cancellation or for lack of attendance.

IMPORTANT: Registrants who do not cancel according to the guidelines above noted forfeit all fees. Refunds will be processed within thirty (30) days following the meeting.
